5th Gear Kids collects personally identifiable information from users at different points on the website. 5th Gear Kids is the sole owner of the information collected on this website which is operated by the Anschutz Health and Wellness Center.
In order to use this Web site, the child must first complete the registration. The information collected from children registering for the website: first name, last name, date of birth, 5th Gear Kids ID card#, school district, school, and parent email. This information is collected directly from the child but with consent from a parent. Children sign up with their information and must provide parent email.
We provide opportunities to participate in contests or surveys on our site. If the child participates, we will request certain personally identifiable information from them. Participation in these surveys or contests is completely voluntary and you and your child therefore have a choice whether or not to disclose this information.
We will not share the personally identifiable information provided through a contest or survey with other third parties unless we give you prior notice and obtain permission.
The personal information provided on this website will be used for monitoring program participation, following contest entries and content submissions, notifying contest winners, and for the distribution of prizes.
Children sign up with their information and must provide parent email. Email is generated from the website to the parent email account prompting the parent to read a COPPA compliance agreement and to approve. Only when the parent has approved a child‘s registration is the information collected into the website database.
